Creighton's Slender Opossum is recorded from wet montane forest in Bolivia's Río Zongo valley. The original description reports a dark chocolate-brown back and a slightly paler underside.
The dorsal fur is rich chocolate brown and the ventral fur slightly paler brown. There is no sharp boundary between the two, and the black eye mask does not extend onto the muzzle.
The known specimens were collected in wet montane forest between 2,000 and 3,000 m.
An extinction-risk assessment has not been added yet.
